Job Summary

Join Acentra Health as a Clinical Assessor in New Hampshire. As a Clinical Assessor at Acentra Health, you will play a key role in serving your community by conducting needs-based eligibility assessments for individuals applying for Medicaid-funded personal care services.These evaluations take place in patients’ homes, nursing homes, and hospitals in Nashua, New Hampshire, and the nearby areas. Your work will be essential to determine appropriate placements and next levels of care for individuals with complex needs, supporting improved long-term health outcomes.

As a Clinical Assessor, you will conduct evaluations that guide care planning, inform placement decisions, and support smooth transitions to the appropriate level of care. This Full-Time hybrid role offers the flexibility of remote work from your home office with some local travel, allowing you to make a meaningful impact within your community. We also reimburse travel-related expenses, including mileage at the federal rate and cover any parking. In addition, we provide all essential technology and equipment for your home office, including a company-issued laptop for both remote and field work.

Job Responsibilities

  • Conduct thorough clinical assessments of patients, reviewing relevant records and consulting collateral sources to ensure accurate evaluations and well-informed recommendations.
  • Recommend appropriate levels of care and service needs based on a comprehensive evaluation of the patient’s physical, behavioral, and psychological functioning.
  • Complete assessments promptly and within the scope of practice, meeting all contractual and regulatory requirements.
  • Demonstrate strong typing and computer skills, with proficiency in Microsoft Word, Outlook, and other relevant software.
  • Adhere to program workflows and ensure compliance with established processes and standards.
  • Perform person-centered assessments, providing meaningful and well-considered recommendations for levels of care and support services.
  • Leverage clinical expertise and knowledge of psychiatric disorders as outlined in the latest Diagnostic and Statistical Manual (DSM) applicable in New Hampshire.
  • Apply knowledge of mental health standards of practice, as well as intellectual and developmental disabilities, geriatrics, and conditions commonly requiring long-term care services and supports.
  • Demonstrates flexibility in adapting to changing priorities, with strong organizational skills and the ability to efficiently prioritize and execute tasks.
  • Leverage technology by utilizing the state's electronic system and our proprietary platforms to efficiently manage and track assessment activities
  • Understand and adhere to all corporate policies, including those related to HIPAA and its Privacy and Security Rules.
  • Maintain compliance with established workflows, regulatory requirements, and best-practice standards.

Qualifications:

Required Qualifications and Experience

  • Must possess an active, unrestricted RN license with eligibility to practice in the state of New Hampshire.
  • Graduation from an accredited college or university with a degree in a relevant field, along with recent hands-on experience conducting clinical assessments, ensuring accurate evaluations, and providing appropriate next level of care recommendations.
  • Proficiency in medical record abstracting, including a strong understanding of medical record organization, medical terminology, and disease processes.
  • Demonstrated ability to communicate clearly and effectively verbally, in writing, and through electronic mediawith both individuals and groups.

  • Detail-oriented with excellent organizational skills and the ability to prioritize and complete tasks efficiently within established timeline
  • Knowledge of public mental health systems and medical programs is preferred.
  • Proficient in computer-based software and Microsoft Office Suite, including Word, Excel, Outlook, and Teams.

Compensation

The compensation for this role ranges from $72,800 to $79,040 base salary. 

Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
